Buckeye Hills Regional Council is seeking a full-time Assessor for our eight-county service area. The Assessor utilizes the skills and knowledge from their profession as an RN or LSW to implement, evaluate, and coordinate various consumer activities within the PASSPORT program.

The essential duties and responsibilities include but are not limited to:

  • Educate consumers on program eligibility, participation, rights, and responsibilities
  • Develop the initial care plan based on enrollee strengths/weaknesses, needs, formal and informal support
  • Authorizes home and community-based services and long-term care options in the consumer’s service plan as supported by the assessment, care plan, and case documentation
  • Determine presumptive Medicaid eligibility and maintain timely assessment documentation.

Essential skills, knowledge, and abilities include but are not limited to:

  • Must possess advocacy and problem-solving skills
  • Manage assigned caseload
  • Prepare accurate & concise reports
  • Determine appropriate treatment or referrals
  • Maintain confidentiality
  • Geriatric health & social issues
  • Basic medical terminology

Education/Experience Required:

Must be a Registered Nurse, Licensed Practical Nurse, or Licensed Social Worker with a valid and current license in the State of Ohio and one (1) year experience.

Starting Salary Range: $37,189 – $46,486.

Fingerprinting, background check, pre-employment drug testing, and a valid driver’s license are required for hire.
