Types of Providers

Learn about the various types of service providers and applicable rules and requirements. Prior to applying for certification, please read and understand the conditions of participation and service requirements for which you are seeking certification. Our library of training videos explains many of these requirements for new providers. Once you’ve reviewed the requirements, you can apply for certification through Ohio’s Department of Aging website.

Types of Providers and Certification Requirements

A legally-organized business: 

  • Employs one or more staff member
  • Registered with the Ohio Secretary of State
  • Received payment for providing services for least 2 adults for a minimum of 3 months for all services and in all counties, you are requesting certification

A legally organized business: 

  • Owned and controlled by 1 person and does not employ any staff 
  • Registered with the Ohio Secretary of State 
  • Received payment for providing services for least 2 adults for a minimum of 3 months for all services and in all counties, you are requesting certification

A licensed residential care facility (RCF) by the Ohio Department of Health:

A registered nurse (RN) or a licensed practical nurse (LPN) at the direction of an RN: 

  • Provide services within the nurse’s scope of practice and shall possess a current, valid and unrestricted license from the Ohio Board of Nursing 
  • Spouses, parents, step-parents, authorized representatives, legally responsible family members, power of attorney, foster caregiver, authorizing health care professional, and legal guardians are ineligible to become a provider

To be eligible for certification, you must have an individual currently receiving services through the PASSPORT Medicaid Waiver Program who has identified you to provide their Choices Home Care Attendant Services. 

  • Spouses, parents, step-parents, authorized representatives, legally responsible family members, power of attorney, foster caregiver, authorizing health care professional, and legal guardians are ineligible to become a provider 
  • Education/Training Requirement: Certificate of completion of five areas of core competency training: basic home safety, universal precautions, promoting the individual’s development, assisting with activities of daily living, communicating the individual’s information to authorized persons, and performing administrative tasks.
